I would delete the following questions:
7,8,9,12,13,14,15,16,17
I  would update wording on 19, and 11.

I would look at adding:
questions about listening at work, re:  whether listening at work is 
restricted by management ,cow-orkers,  what time you arrive at work, 
when do you leave work.
When do you leave for work and how long is your commute?
